What to include in your message

The more of these you can answer, the sooner the search starts. Skip anything you are unsure of; your advisor will ask.

Your NAU track: MS FNP, MS PMHNP, the post-master's FNP or PMHNP certificate, or the DNP.
The course ahead (for FNP, NUR 661, NUR 663 or NUR 665; for PMHNP, NUR 680) and the term it starts.
If a practicum is already running, the hours you have logged and the hours still to go.
Your town, the longest commute you could manage on clinic days, and your state if you live outside Arizona.
Where you work, and whether a site there is worth exploring.
Any preceptor leads you already have, and any clinics that have already turned you down.
Where your Green Form stands: not yet filed, filed asking NAU for help, or approved and then lost.

Leave health records and copies of ID out of this first message. When a particular site needs a document, your advisor asks for that one item and tells you why.

What happens after you send it

You are welcome to write with a single question and nothing more. Plenty of students first get in touch simply to understand how NAU's Green Form and approval steps fit together before they decide anything about a search.

If a deadline is close

Put it in the first line: "Green Form due in two weeks", "documents due next month" or "preceptor cancelled". NAU's 2022-23 form lists cutoffs of March 31, June 30 and October 31 for summer, fall and spring, and NAU expects your clinical paperwork filed a full two months before the rotation, so a few days can matter. Tell us which date you are racing, and whether your own documents are already filed.

Preceptors, clinics and practice managers

Clinicians who would like to host an NAU nurse practitioner student are welcome here too. Set the program field to "I'm a preceptor (not a student)", then describe your license and specialty, the ages and conditions your practice sees, your clinic's town and the days a student could join you.

Office managers can write as well, particularly about NAU's affiliation agreement, which a practice signs before a student starts. If your clinic has hosted NAU students before, say so; an agreement already on file can shorten the timeline considerably. When will I hear back?

Replies go out by email, phone or text, whichever you asked for, and anything flagged with a near Green Form cutoff or a cancelled preceptor jumps the queue. If you are up against a date, name it in your opening line so your advisor sees it first.

Is it fine to write with just a question?

Yes. Many students start by asking how NAU's process works, or whether a clinician they know would qualify under Arizona's preceptor rule. Your advisor answers plainly and explains what a placement plan would involve, and you decide from there, with no commitment on your side.

Does this form reach NAU?

No. It goes to our independent placement desk only. For NAU matters, contact your graduate nursing advisor, your course faculty or your program coordinator. Your Green Form travels from you to NAU directly, as NAU's own instructions set out, and our desk prepares what goes on it.

Who sees the information in my message?
